Incident Investigation Training ICAM must occur when any unsafe work takes place whether a serious injury occurs or not.  You must record all incidents in a register of injuries and investigate why the incident occurred.

You must document:

  1. the investigation,
  2. the outcome,
  3. all work changes or risk controls put in place as a result, and
  4. how you have communicated these changes to your workers.

What is ICAM?

There is a system of incident investigation called, ICAM, Incident Cause Analysis Method, an industrial safety initiative that draws on the work of the eminent organisational psychologist Professor James Reason.  Our training covers the ICAM system of investigation as well as the five “Whys”.

Our incident investigation training provides you or your staff with the skills, knowledge required to plan, conduct and report on investigations of work health and safety (WHS) incidents that have resulted in, or have the potential to result in, injury or damage.

We provide three levels of training:

  • Incident Investigator
  • Lead Incident Investigator
  • Incident Investigation Governance
